We welcome referrals from professionals who support adults to live safely and independently at home.

Support we can provide

We provide personal care and practical support at home for adults over and under 65. Every care plan is built around the person's routine, preferences and goals.

  • Personal care, including washing, dressing and continence support
  • Help with meals, drinks and daily routines
  • Companionship and support to stay connected
  • Domestic support and help around the home

We do not provide nursing or complex clinical care. If you are unsure whether we can meet a person's needs, please get in touch and we will talk it through with you.

How the referral process works

    Initial contact

    Send a referral request using the form below or call our team. Share only the basic details we ask for.

  2. 2

    Referral discussion

    We contact you to discuss the person's needs, the care required, timescales and funding arrangements.

  3. 3

    Secure sharing of assessments

    If we are able to help, we agree a secure way to share assessments, care plans and any other relevant documents.

  4. 4

    Assessment and start

    We complete our own care assessment with the person and agree a start date, subject to carer availability.
